To request a spot at a free community clinic day with Positive Tails at Flatbush Vet, please fill out the form below below. Be sure to add each of the animals you’d like to bring up to a maximum of 4 per family.

Doctors will be on hand to give each animal an exam, basic vaccinations, and flea/tick/heartworm preventives.

While you will be given the opportunity to select your preferred time slot, please be aware that this will be a busy day and your patience is appreciated. Make sure to bring what you need for all two-legged and four-legged members of your party to stay warm as you wait!
